Growing Health Together are excited to have been featured as a case study within an NHS Confederation report. The report details the ongoing Growing Health Together story, demonstrating the work that we have done in Neighbourhoods, from conception to now, and how we have taken a different approach and a new model to support community-led health creation and improvements in clinical care, through connecting primary care with community members and partners at a neighbourhood level.
