In December 2020 Surrey Heartlands Clinical Commissioning Group invited Growing Health Together to assist groups of GP surgeries called Primary Care Networks, to collaborate with community members and local organisations in order to improve health and prevent disease across East Surrey.

OUR PRIORITIES

HEALTH

Supporting social, mental and physical health for people of all ages and backgrounds as we re-emerge from COVID-19 restrictions.

EQUITY

Making access to health-giving opportunities, such as fresh food and the means to be physically active, fairer and more equal.

SUSTAINABILITY

Reducing waste and supporting a healthy natural environment, recognising this is critical to human health.

HOW WE WORK

Collaborative partnerships are developing in Horley, Caterham, Merstham, Redhill, Oxted and surrounding areas, roughly corresponding to the geography of our five East Surrey Primary Care Networks (PCNs). Each area is working on different projects to support health and wellbeing, reflecting the different interests, needs and capabilities within each place, but common principles of work in each PCN include:

Identifying and highlighting existing health and wellbeing opportunities, outside of the health service, within each locality, so that more people might benefit from them.

Listening to local people’s views on the sources of and barriers to health in their communities.

Supporting and enabling community members to lead their own health creation activities.

Facilitating collaboration between citizens and local organisations in order to co-create better conditions for health and wellbeing in our communities.
