Other news from Care Collaborative PCN

• Conversations have continued between the Community Midwife for Surrey and Sussex hospitals (SASH), local GHT leads, Surrey Heartlands and local people, in order to establish breast-feeding support into Merstham either in Merstham Hub or Red Oaks Wellbeing Centre.
• Tabassum invited Anita Lamb and Elana Shelley from First Community Health and Care (FCHC) and the 0-19 team respectively to agree closer working and support from the practice for the community and patients. This will include providing access/signposting to available services and working with local schools to support identified needs.
• Tabassum has been working closely with PCN network co-ordinator Melanie Hall to establish outreach smoking cessation clinics. There has been fantastic uptake of this service, in collaboration with One You Surrey | Free Healthy Lifestyle Service
